About The Position

The Spiritual Counselor promotes consistent, high-quality patient care by functioning within the Coastal Hospice IDT model to meet patient/family goals and maintain effective relationships with contracted facilities. This role involves providing assessment, planning, intervention, and ongoing evaluation of spiritual needs to meet assigned patient and family goals. The counselor will collaborate with the Interdisciplinary Team (IDT) and participate in family meetings to address patient/family needs, offering spiritual support to alleviate suffering and facilitate acceptance of the dying process. Additionally, the role involves utilizing community resources, coordinating with area clergy, and referring patients/families to outside agencies when necessary. The Spiritual Counselor is responsible for maintaining continuity and consistency of care through accurate electronic medical record documentation, communicating essential information to colleagues, and assisting with bereavement services as per policy. The position also includes developing and providing educational programs to staff, volunteers, faith groups, and community organizations, as well as offering spiritual support to Coastal Hospice staff.

Requirements

Master’s degree in divinity or theology or pastoral counseling from an accredited seminary or school of theology preferred.
Current ecclesiastical endorsement preferred.
Completion of a minimum of one (1) unit of Clinical Pastoral Education (CPE) within prescribed amount of time or equivalent clinical training and experience preferred.
Demonstrated ability in facilitating individual and group support.
Sensitivity to the particular needs of those in grief.
Ability to safely and successfully perform the essential job functions consistent within the guidelines of ADA, FMLA, OSHA and other federal, state and local laws.
Must be able to lift and carry up to fifteen (15) pounds with or without the use of assistive devices.

Responsibilities

Provides assessment, planning, intervention, and ongoing evaluation of spiritual needs to meet assigned patient and family goals.
Attends and actively participates in scheduled meetings of the designated IDT.
Collaborates with members of the IDT on ways to support the patient and/or family.
Participates in family meetings to address patient/family needs.
Provides spiritual support to patients/families to alleviate suffering and facilitate acceptance of the dying process.
Utilizes community resources to assist the patient/family to meet goals.
Coordinates with area clergy and religious leaders to provide spiritual care to hospice patients and families.
Refers patients and/or families to outside agencies, if necessary.
Completes spiritual documentation in the patient’s electronic medical record within the prescribed time frame.
Triages essential information to other colleagues to communicate spiritual needs as it relates to transitions in patients care.
In collaboration with Bereavement Services, assists with providing follow-up bereavement calls, required visits and initial assessments per policy.
Develops and provides educational programs to Coastal Hospice staff and volunteers, local faith groups and community organizations, as requested.
Provides spiritual support to Coastal Hospice staff, as requested and appropriate.
