About The Position

The Spiritual Care Coordinator provides direct spiritual care to patients and families as requested. This role actively participates in the coordination of all aspects of patient care, contributing to patient and family counseling and education. The coordinator is involved in ongoing interdisciplinary comprehensive assessments and documents spiritual care interventions and outcomes according to organizational standards. This position also coordinates the UH Clergy Advisory Board, contacts community clergy on behalf of patients/families, and assists in the development and planning of hospice-related events with a spiritual component. Additionally, the role supports volunteer and staff orientation, in-services, training, and supervision, and participates in QAPI programs and hospice-sponsored training. The coordinator may also participate in or conduct memorial services or funerals and assist in bereavement activities. A key aspect of this role is maintaining Point of Service documentation and incorporating the organization's commitment to customer service and continuous quality improvement into daily work. Proficiency in Microsoft Office, EMRs, and the ability to learn new software are required, as is access to a car, valid driver's license, and current insurance.

Requirements

  • Graduate of an accredited seminary or school of theology or advanced training in pastoral care/counseling
  • Knowledge of emotional effects of death, grief and loss
  • Ability to be open, flexible and ecumenical in ministry
  • Commitment to the hospice concept and its implementation
  • Must have home internet and email access and be proficient in Microsoft Windows
  • Ability to relate to a diverse population and workforce in a personable and effective manner

Responsibilities

  • Provides direct spiritual care to patients and families as requested by patient/family
  • Actively participate in coordination of all aspects of patient’s care, in accordance with current professional standards and practice
  • Contributes to patient and family counseling and education
  • Participates in ongoing interdisciplinary comprehensive assessment
  • Documents spiritual care assessments, interventions and outcomes in an appropriate and timely manner following UH documentation standards
  • Coordinates the UH Clergy Advisory Board
  • Contacts community clergy on the patient/family's behalf
  • Assists in the development/planning of hospice related events which contain a spiritual component
  • Assists in volunteer and staff orientation, in-services, training and supervision as appropriate
  • Participates in QAPI program and hospice sponsored in-service training
  • Participates or conducts memorial services or funerals for patients as requested
  • Assists in bereavement activities as appropriate
  • Maintains Point of Service documentation at a minimum rate of 80%
  • Accepts other responsibilities as assigned
