We are seeking an experienced Spiritual Care Coordinator to join our team and provide compassionate spiritual support throughout the South Hills and City of Pittsburgh areas, with travel throughout our service area as needed. Requirements

  • Bachelor's degree from an accredited institution in Religious Studies, Theology, Pastoral Care, or a related field required.
  • Must have a valid Pennsylvania driver's license.
  • Travel throughout the Bridges Hospice service area is an essential function of the position.
  • Must be able to communicate effectively in English, both verbally and in writing.

Nice To Haves

  • Clinical Pastoral Education (CPE) training preferred.
  • Previous experience providing spiritual counseling or pastoral care in a healthcare, hospice, hospital, or similar setting preferred.
  • Previous hospice experience highly preferred.
  • Strong interpersonal and customer service skills.
  • Excellent verbal and written communication skills.
  • Strong critical-thinking, organization, and problem-solving skills.
  • Ability to work effectively as part of an interdisciplinary team.
  • Demonstrated ability to work respectfully with individuals from diverse religious, spiritual, and cultural backgrounds.

Responsibilities

  • Provide spiritual counseling, emotional support, and guidance to patients and families based on their individual beliefs, values, and preferences.
  • Assess and address the spiritual and emotional needs of patients and families as part of the hospice interdisciplinary team.
  • Collaborate with members of the interdisciplinary team to develop and update individualized patient care plans.
  • Support patients and families from a variety of religious, spiritual, and cultural backgrounds with sensitivity and respect.
  • Collaborate with the patient's clergy, faith community, or other spiritual support persons as appropriate and with patient/family consent.
  • Consult and collaborate with the Bereavement Coordinator and assist with bereavement services as appropriate.
  • Facilitate or coordinate spiritual support groups and community programs as appropriate.
  • Participate in interdisciplinary group meetings and communicate relevant patient and family needs.
  • Maintain accurate, timely, and complete clinical documentation in accordance with organizational and regulatory requirements.
  • Provide spiritual support during times of crisis, decline, and end-of-life transition.
  • Advocate for patient and family spiritual needs while respecting individual beliefs, values, and choices.
