About The Position

The Speech Therapist will perform patient assessments, including appropriateness for home care, learning needs, and progress toward goals. They will collaborate with patients, families, and physicians to develop care plans and assess swallowing, communication, and cognitive abilities. The role involves implementing treatment programs to achieve designated goals, administering services per physician orders, and instructing in communication, safe swallowing, and cognitive training. The therapist will also develop and instruct in therapeutic exercise programs, implement programs to restore function, and provide discharge planning and referrals. Communication with physicians, nurses, and other clinical disciplines is essential, as is developing and evaluating care plans based on evidence-based knowledge and revising them as needed. The position requires using age-appropriate teaching methods for patients and families, evaluating and revising teaching plans, and documenting patient education tools. The therapist will assume responsibility for case management, demonstrate knowledge of home care regulations and insurance reimbursement guidelines, and ensure compliance with regulatory requirements and agency documentation policies. Evidence of interdisciplinary communication, appropriate utilization of services, and adherence to the scheduling process are expected. Maintaining visit productivity standards, participating in Team and IDT meetings, and prioritizing schedules to accommodate patient needs are key responsibilities. The role involves traveling to patient homes and facilities, complying with paid time off requests, and meeting weekend/holiday requirements. Additionally, the therapist will assist with growing agency business by providing orientation to new employees, accepting other assignments, and cross-training to learn new skills. Use of a personal vehicle is required for home visits.

Requirements

Minimum of one (1) year of experience in acute, subacute or home care setting.
Good customer service and communication skills.
Graduate of an accredited speech and language program with a minimum of a master's degree.
Current NJ ST License.
Use of personal vehicle is required for home visits.

Responsibilities

Performs Patient assessments: Accurately assesses patient's appropriateness for home care, learning needs and progress toward goals.
Collaborates with patient, family, and physician to develop the plan of care for the patient.
Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated.
Maintains safe environment for patients and co-workers: Assesses social environment and adjust or provide education as needed.
Assess for potential clinical risks and adapt program as indicated.
Follows universal infection control measures.
Implements treatment program to achieve designated goals: Administer occupational therapy services per physician orders.
Instruct in communication and safe swallow, cognitive training as indicated.
Develop and instruct in therapeutic exercise program.
Implement program to restore function, and achieve maximum functional status.
Provides discharge planning and referral to resources as needed.
Communicate with physician, nurse, and other clinical disciplines as necessary.
Develops/implements/evaluates plan of care based on evidence-based knowledge.
Revise care plan as indicated within physician orders.
Uses age appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary.
Documents patient education tools utilized.
Demonstrates compliance with agency policies: Assumes responsibility for Case Management.
Demonstrates knowledge of home care regulations and insurance reimbursement guidelines.
Assures compliance with regulatory requirements.
Follows agency documentation policies.
