About The Position
Hometown Home Health is a full-service home health care agency providing skilled medical home care services and clinical nursing care. The Speech Therapist provides personalized speech pathology care to patients in their homes, including assessments, evaluation of functioning, and treatment planning in collaboration with patients and their primary physicians. The role also supports patient care plan development and implementation and documents visit progress in patient charts.
Requirements
- Master’s or doctoral degree in speech-language pathology.
- Licensed to practice in the state of Florida.
- CPR certification; online certification is not accepted.
- At least one year of institutional experience.
Nice To Haves
- Membership in the American Speech-Language-Hearing Association is preferred.
Responsibilities
- Provide speech pathology services and assessments to patients in their homes.
- Evaluate patients’ level of functioning and recommend mechanisms to enhance patient communication.
- Establish a speech therapy treatment plan with the patient and primary physician.
- Assist in the implementation and development of patient care plans.
- Prepare progress notes following visits for patients’ charts.
