About The Position

Guardian Angel Home Care is seeking compassionate, motivated, and dedicated Speech-Language Pathologists to provide home health care nationwide. The Speech Therapist helps patients improve communication, cognitive function, swallowing abilities, and quality of life while receiving care at home. The role works closely with physicians, nurses, therapists, patients, and caregivers to develop and implement individualized treatment plans that promote independence and successful outcomes.

Requirements

Master's Degree in Speech-Language Pathology from an accredited program.
Current Speech-Language Pathologist license in the state of practice.
Current CPR certification or willingness to obtain it.
Valid driver's license, reliable transportation, and automobile insurance.
Strong communication, organizational, and time management skills.

Nice To Haves

Certificate of Clinical Competence (CCC-SLP).
Home health experience.

Responsibilities

Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
Develop individualized treatment plans based on physician orders and patient needs.
Provide skilled speech therapy services in accordance with professional standards and agency policies.
Assess and treat communication, cognitive-linguistic, and swallowing impairments.
Educate patients, families, and caregivers about treatment plans and home exercise programs.
Collaborate with interdisciplinary team members to ensure coordinated patient care.
Complete timely and accurate clinical documentation.
Participate in case conferences and care planning meetings as needed.
Monitor patient progress and adjust treatment plans accordingly.
Maintain compliance with federal, state, and agency regulations.
