Requirements

  • Must have a current SLP license (state-specific).
  • Graduate of an accredited speech language pathology program.
  • Valid BLS certification.
  • Minimum two years of speech therapy experience (home health experience preferred).
  • Excellent communication, organizational, and time management skills.
  • Must have a valid driver’s license and access to a reliable and insured vehicle.
  • Ability to visit homes that may not be wheelchair accessible.
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
  • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.

Nice To Haves

  • Home health experience preferred.

Responsibilities

  • Evaluate patients’ speech, language, and hearing abilities, recommending strategies to enhance communication and hearing.
  • Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
  • Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
  • Communicate regularly with physicians, addressing any changes in patient condition.
  • Participate in the periodic revision of care plans, updating the physician and care team on patient status and obtaining additional orders as required.
  • Educate patients and families/caregivers on speech therapy techniques and related challenges.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Document all evaluation data, treatments, and patient progress promptly.
  • Supervise home health aides every 14 days when speech therapy is the sole discipline involved.
