About The Position
Health Link Home Health and Hospice is seeking a compassionate, organized, and team-oriented individual to fill the role of Per Diem Licensed Speech Therapist (ST) for the Santa Clara County. You will work closely with our multi-disciplinary team to ensure coordinated patient care. Our clinical and office staff in San Francisco work together harmoniously to deliver outstanding care. Requirements
- Current ST License to practice in California - Required
- Current CA Driver's License
- Coverage area: Santa Clara County
Nice To Haves
- Home Health: 1 year - Preferred
Responsibilities
- Speech Therapists will provide services within the scope of speech therapy, in accordance with the plan of care individualized for the patient, and in coordination with the Interdisciplinary team
- Follow the plan of care; provides treatment; evaluate patient's reaction to treatment and report any changes in his/her condition to the Physician, Case Manager and/or or Director of Nursing
- Maintain appropriate clinical records and/or notes and submit the records with organization policy
- Be responsible for following all company policies and procedures
