About The Position

Aspire Home Healthcare is seeking a skilled and compassionate Speech Language Pathologist (SLP) to join their team in San Diego, CA. This role involves providing quality, one-on-one care to patients in their homes, helping them to thrive. The SLP will assess, diagnose, and treat speech, language, and swallowing disorders, developing personalized treatment plans to support rehabilitation goals and promote independence and enhanced communication skills. Requirements

Licensed Speech Language Pathologist (SLP) in the State of California
Minimum of 1 year of SLP experience preferred
Current physical (within the last 6 months)
Current TB test (within the last 12 months) and CPR certification
Valid driver’s license and reliable transportation
Ability to attend a 1-day orientation at our San Diego office

Responsibilities

Provide effective patient care using approved therapy techniques and procedures
Evaluate patients’ functional levels, treatment needs, and progress toward goals
Develop and implement individualized treatment plans in accordance with physician orders
Supervise Speech Therapy Assistants (STAs) and coordinate patient care
Document treatments, progress notes, and care plan updates accurately and on time
Assist patients and families in developing and reviewing home programs
Recommend adaptive equipment and evaluate home settings when needed
Maintain patient confidentiality and compliance with HIPAA regulations
Collaborate with other healthcare providers to ensure coordinated, high-quality care
Complete discharge summaries and obtain necessary orders at case completion
