About The Position

Hillcrest Health & Living is looking for a compassionate and skilled Speech Therapist to join our Hillcrest Home Care team on a PRN basis. Hillcrest Home Care supports individuals in the comfort of their own homes, helping them regain function, improve safety, and maintain as much independence as possible. Our team partners closely with patients, families, providers, and other care professionals to deliver personalized care that supports each individual’s goals. Requirements

  • Graduate of an accredited Speech Therapy program required.
  • Active Speech Therapist license in Nebraska or a compact state required.
  • Basic Life Support CPR certification required or willingness to obtain within 45 days of hire.
  • Must be able to speak, read, and write English.
  • Must be able to independently lift up to 35 pounds.
  • Must be able to perform repetitive sitting, bending, standing, walking, and movement as needed.
  • Must be flexible, calm, and professional in stressful or changing situations.
  • Must be able to work in a fast-paced environment with strong attention to detail.
  • Must be able to utilize technology and clinical resources efficiently and effectively.

Nice To Haves

  • Experience as a Speech Therapist in home care, long-term care, post-acute care, hospital, or a similar setting preferred.
  • Strong evaluation, treatment planning, and clinical documentation skills.
  • Ability to support patients with communication, cognition, swallowing, voice, and fluency needs.
  • Excellent communication and relationship-building skills with patients, families, providers, and team members.
  • Strong attention to detail and commitment to quality outcomes.
  • Ability to work independently while staying connected to the interdisciplinary team.
  • A calm, flexible, and professional approach in changing or stressful situations.
  • Comfort using technology and clinical resources efficiently and effectively.
  • At least two years of experience as a Speech Therapist in long-term care, post-acute care, hospital, home care, or a similar setting preferred.

Responsibilities

  • Complete evaluations using appropriate tests and measures to establish timely, individualized plans of care.
  • Provide therapy services in accordance with each patient’s plan of care, clinical needs, and established goals.
  • Treat and support needs related to speech, language, cognition, communication, voice, swallowing, and fluency.
  • Educate patients and family members using clear communication, adult learning principles, and sensitivity to individual needs.
  • Collaborate with physicians, nurses, therapists, caregivers, and other interdisciplinary team members.
  • Monitor patient progress and modify treatment plans to support expected outcomes.
  • Complete accurate documentation and charging to support therapy services.
  • Plan safe and effective transitions of care, including equipment recommendations, service needs, and handoff communication.
  • Identify patients who may benefit from additional medical or therapy services and follow through appropriately.
  • Build an efficient patient schedule while considering patient preferences, home environment, care needs, and productivity standards.
  • Identify and address risks related to patient care, infection control, documentation, equipment, and safety.
  • Participate in professional development, clinical education, and other activities that support the therapy profession.
