Requirements
- Must have a current drivers license
- Ability to travel to case assignments as assigned.
- Ability to work independently with minimal supervision and manage time effectively
- Strong interpersonal skills.
- Current license as a Speech Therapist in Pennsylvania
Responsibilities
- Visit patient and assess patient, care and plans with patient/family and physician.
- Assist the patient with activities of daily living and facilitate their efforts towards self-sufficiency and independence
- Teaches and supervises patient and family in the home environment regarding treatment, self-care, coping and preventive health measures.
- Communicates effectively with patient, patient’s family, agency staff, physician, and other involved disciplines (i.e., social services, therapists, etc.) to carry out treatment plan objectives.
