About The Position

Concordia Visiting Nurses is a clinician-led home health agency seeking a personable and motivated Speech Therapist (ST) to join their growing team. The role involves working one-on-one with adult and geriatric patients in their homes to help them improve communication, swallowing function, and daily confidence. Requirements

• Current license as a Speech-Language Pathologist in PA
• ASHA CCC-SLP Certification
• 1+ years of experience (home health a plus but not required—we’ll train the right candidate!)
• Strong interpersonal and communication skills
• Reliable transportation and valid driver’s license

Responsibilities

• Provide skilled speech therapy services to patients in their homes
• Develop and implement personalized care plans
• Evaluate speech, language, cognition, voice, and swallowing disorders
• Educate patients and caregivers on therapeutic strategies
• Collaborate with nurses, PT/OTs, and physicians to ensure the best outcomes
