The Speech Therapist will evaluate patients within twenty-four (24) hours or 1 business day of physician referral. If unable to complete the evaluation in 24 hours, documentation in the medical record must clearly define the cause for the delay. The therapist will develop an effective treatment plan and obtain approval for services from the referring physician, and then treat patients per the physician's treatment plan. This role involves supervising Speech Language Pathologist Assistants in direct patient care and patient-related activities, following the state practice act. The therapist will communicate with supervisors and other health team members regarding patient progress, problems, and plans. Participation in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences is expected as needed. The role also includes participating in in-service training programs for other staff in the facility. Recording evaluations, daily treatment notes, weekly progress notes, and recertification documentation is required per the Speech and Language Pathologist in the Board State Practice Act and governmental and third-party payer requirements. Treatment changes must be recorded per policy and procedures. The therapist will instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services, and participate in discharge planning. Additionally, the Speech Therapist will act as a clinical preceptor for affiliating CFY/RFP Speech Language Pathologists and student programs. Compliance with the Speech Language Pathology and Audiology Bureau State Practice Act and facility policies and procedures is mandatory. The ability to relate positively, effectively, and appropriately with patients/residents, families, staff, and professional colleagues is essential.
