About The Position
Human Touch Home Health is seeking a passionate Speech-Language Pathologist (SLP) to join our growing team serving patients throughout Fairfax County, Loudoun County, and Prince William County. This PRN opportunity offers flexibility, autonomy, and the ability to make a life-changing impact one patient at a time. As a Home Health Speech-Language Pathologist, you'll help patients overcome challenges related to speech, language, cognition, communication, and swallowing while improving their quality of life in their homes. Every visit is an opportunity to help someone communicate more clearly, eat more safely, think more confidently, and regain control over their daily life.
Requirements
- Active Virginia Speech-Language Pathologist License Required
- Valid Driver's License and Reliable Transportation
- Strong Communication and Documentation Skills
Nice To Haves
- Certificate of Clinical Competence (CCC-SLP) Preferred
- Home Health Experience Preferred but Not Required
- Passion for Delivering Exceptional Patient-Centered Care
Responsibilities
- Complete speech, language, cognitive, and swallowing evaluations
- Develop individualized treatment plans based on patient needs and goals
- Provide skilled speech therapy services in patients' homes
- Educate patients and caregivers on communication and swallowing strategies
- Collaborate with physicians and interdisciplinary team members
- Monitor patient progress and adjust treatment plans as needed
- Maintain timely and accurate clinical documentation
