About The Position

Responsible for the delivery of speech therapy services to patients in accordance with an established physician plan of care and in compliance with applicable licensure/certification requirements, Texas state practice act, and the agency's policies and procedures. Due to the nature of hospital services, it may become necessary to work extended hours.

Requirements

• Master's Degree in Speech Pathology from an accredited college and an Certificate of Clinical Competence in speech and language pathology.
• Current licensure in speech pathology as required by the state of Texas.
• Specialized training in MBS procedures and evidence of competency in performing modified barium studies preferred.
• Effective verbal and written communication skills.
• Ability to meet minimum hiring standards.
• Must be able to meet the physical demands which include standing, Walking, bending, lifting, and transferring; may involve lifting or positioning of patients.
• Must be able to utilize good judgment, demonstrate patience, and maintain a professional demeanor.
• Must be able to sustain concentration in a busy and stressful environment.
• Must work varied hours and schedules.
• Requires corrected vision and hearing to normal range.
• May be exposed to communicable diseases and bodily fluids.
• May be exposed to violent or disturbed individuals.
• Demonstrates an understanding of and adherence to the HMHD Compliance Plan.

• Attends the required corporate integrity and compliance training and education programs.

Nice To Haves

• Two or more years of clinical experience desired.

Responsibilities

• Delivers direct speech therapy services to patients as ordered by a physician and in accordance with an established plan of care.
• Evaluates the patient within 72 hours.
• Plans and develops appropriate speech therapy care plan based on evaluation and as approved by the attending physician.
• Documents patient evaluation on appropriate forms in a timely manner.
• Develops a plan of care and consults with physicians and other members of the healthcare team on an ongoing basis for updates and communication.
• Documents short-term and long-term goals and includes discharge planning activities.
• Includes appropriate staff and family members in goal planning to provide continuity of care.
• Utilizes community resources and agency personnel by proper referral methods.
• Recommends communication devices and aids as indicated and according to patient capability.
• Administers speech therapy program utilizing specialized therapeutic techniques and/or equipment.
• Documents progress in meeting goals and/or documents reasons for failure to meet goals.
• Maintains and submits all required patient documentation relative to care delivered to the patient.
• Documents after each visit.
• Maintains appropriate documents required for billing.
• Confers with hospital staff and family members regarding methods of assisting patient to improve, correct and/or accept level of disability.
• Contacts physicians as necessary and appropriate concerning treatment and progress of patient.
• Demonstrates knowledge of and compliance with all hospital policies related to departmental operations.
• Complies with all local, state and federal regulations and JCAHO guidelines regarding speech-language therapy services and attends staff meetings as requested.
• Conducts staff in-services' as requested.
• Attends ongoing continuing education programs and in-services.
• Participates in performance improvement activities as requested.
• Uses critical thinking skills to determine evidence based solutions.
• Systematically gathers and reviews pertinent information to prevent errors (PDCA -- Plan, Do, Check, Act).
• Pursues individual or team learning and growth opportunities to continually build skills.
