About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Speech Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Rancho Cucamonga and the surrounding areas. The role offers flexibility in scheduling and coverage areas, independence as your own boss, and strong per-visit rates allowing you to determine your earnings. FeldCare Connects provides administrative support including assigning, communication, scheduling, care coordination, and quality assurance. Clinicians provide one-on-one treatment, observe and report patient responses, and determine the plan of care. Documentation is made easy with an intuitive Electronic Medical Record system. The role offers a purpose-driven mission to help people and a streamlined referral process through the Cliniconnects app.

Requirements

• Ability and enthusiasm in working with all ages.
• Current Speech Language Pathology license issued by the state Board of Speech-Language Pathology and Audiology.
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Certification by the American Speech-Language and Hearing Association (ASHA) as a Speech Language Pathologist.
• Current Certification of Competence (CCC) is required.
• Completion of an accredited Speech Language Pathologist program.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
