About The Position

Speech-Language Pathologist (SLP) Chandler Creek Post Acute – Greer, SC $45–$50/hour | PRN | Flexible Scheduling About Us Chandler Creek Post Acute is a 133-bed skilled nursing facility located in Greer, South Carolina. We are seeking a skilled and motivated Speech-Language Pathologist (SLP) to join our rehab team on a PRN basis. Your expertise will play a key role in improving communication, cognition, and swallowing function for our residents.

Requirements

Master’s or Doctoral degree in Speech-Language Pathology
Active, unencumbered SLP license in South Carolina (required)
Strong knowledge of speech, language, cognitive, and swallowing disorders
Excellent communication and interpersonal skills
Ability to work independently and as part of a team

Nice To Haves

Certificate of Clinical Competence (CCC-SLP) preferred
Experience in a skilled nursing or rehab setting preferred
Familiarity with PointClickCare (PCC) and Casamba preferred

Responsibilities

Evaluate residents within 24 hours (or 1 business day) of physician referral
Develop and implement individualized treatment plans with physician approval
Provide therapeutic interventions based on patient needs and goals
Monitor patient progress and adjust treatment plans accordingly
Maintain accurate and timely clinical documentation
Participate in care conferences, discharge planning, and interdisciplinary meetings
Educate staff and caregivers on therapy techniques and maintenance programs
Supervise Speech-Language Pathologist Assistants (SLPAs) as applicable
Conduct in-service training for facility staff
