Requirements

  • Master's Degree in Speech-Language Pathology from an accredited program
  • Current Speech-Language Pathologist license in the state where services are provided
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred or required based on state regulations
  • Minimum of two (2) years of experience as a Speech-Language Pathologist
  • Home health or community-based experience preferred
  • Experience evaluating and treating patients with communication, cognitive, and swallowing disorders
  • Strong clinical assessment, communication, and documentation skills
  • Commitment to maintaining HIPAA compliance and patient confidentiality

Responsibilities

  • Evaluate speech, language, communication, cognitive, voice, and swallowing disorders
  • Develop individualized treatment plans based on physician orders and patient needs
  • Provide evidence-based therapeutic interventions to improve communication and swallowing function
  • Assess and treat patients experiencing speech, language, cognitive, and dysphagia-related challenges resulting from illness, injury, or neurological conditions
  • Educate patients, family members, and caregivers on treatment strategies and home exercise programs
  • Recommend communication aids, adaptive devices, and compensatory strategies as appropriate
  • Monitor patient progress and modify treatment plans as needed to achieve therapeutic goals
  • Collaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate care
  • Communicate changes in patient condition and treatment response to the physician and Director of Nursing
  • Complete accurate clinical documentation, progress reports, and discharge summaries in a timely manner
  • Treat clients, families, caregivers, and team members with professionalism, courtesy, and respect
