About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Speech Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Hemet and the surrounding areas. FeldCare Connects is an app-based referral network that connects clinicians to patients in their community. By contracting with home health agencies, they enable patients to access necessary care. They aim to reduce the administrative burden on agencies and clinicians, allowing them to focus more on patient treatment and improve outcomes.

Requirements

• Ability and enthusiasm in working with all ages.
• Current Speech Language Pathology license issued by the state Board of Speech-Language Pathology and Audiology.
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Certification by the American Speech-Language and Hearing Association (ASHA) as a Speech Language Pathologist.
• Current Certification of Competence (CCC) is required.
• Completion of an accredited Speech Language Pathologist program.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
• Complete documentation using an intuitive Electronic Medical Record system.
