Speech Pathologist - Queens NY
About The Position
Queens NYCCC is seeking the services of a licensed Speech-language Pathologist for children/adolescents. Services include patient evaluations, treatment planning, speech therapy, and staff consultation. Documentation must adhere to Uniform Case Record and NYCCC policies. Service hours are a maximum of 20 hours per week and 1040 hours annually. The facility operates Monday-Friday, 8:30 am to 5:00 pm, with flexible scheduling. Mandated training, including HIPAA/HITECH, must be completed. Staff are expected to wear identification badges and integrate into the treatment team.
Requirements
- Valid license to practice Speech-language Pathology in the State of New York.
- Direct clinical experience working with children/adolescents.
- Certified as a TSSH or TSSLD for the State of New York.
- Provide a copy of a valid license.
- Provide updates to license upon request.
- Adhere to all requirements, regulations, and ethical practices, standards, and licensing requirements.
- Provide resumes and mandatory forms prior to interviewing.
- Provide proof of immunizations including PPD/Tuberculosis testing, and the flu vaccine.
- Complete the first four of the five mandatory forms required for clearance prior to interview.
- Complete the Health Clearance Form (for selected candidate).
