About The Position

The Speech-Language Pathologist (SLP) evaluates, develops, and implements individualized treatment plans for residents in a Skilled Nursing Facility (SNF) with communication, cognitive-linguistic, voice, and swallowing disorders. The SLP works collaboratively with the interdisciplinary care team to maximize residents' functional independence, safety, quality of life, and ability to participate in daily activities.

Requirements

  • Professional degree in speech language pathology required.
  • Masters degree in speech pathology from an accredited program.
  • Eligible for Speech-Language Pathologist license to practice in the state of WI.
  • Must hold and maintain Wisconsin Speech-Language Pathologist license.
  • Must maintain a valid, current driver's license.
  • Must maintain BLS certification.
  • Exceptional accuracy and attention to detail required.
  • Intermediate proficiency with computers is required.
  • Strong assessment and planning skills.
  • Strong quantitative and analytical competency.
  • Self-starter with excellent interpersonal communication and problem-solving skills.

Nice To Haves

  • 1+ years of related work experience preferred.
  • Experience in a hospital setting a plus.

Responsibilities

  • Evaluate and treat residents with speech, language, cognitive, voice, and swallowing disorders.
  • Complete evaluations, treatment plans, progress notes, and discharge documentation.
  • Assess and manage swallowing disorders, including diet and safety recommendations.
  • Develop and update individualized treatment plans based on resident needs and goals.
  • Participate in care conferences and interdisciplinary team meetings.
  • Collaborate with physicians, nursing staff, therapists, dietitians, and other healthcare team members.
  • Educate residents, families, and staff on treatment recommendations and swallowing precautions.
  • Provide in-service training and education to facility staff.
  • Monitor resident progress and adjust treatment plans as needed.
  • Support discharge planning and transitions of care.
  • Maintain compliance with facility policies, regulatory requirements, and documentation standards.
  • Maintain treatment equipment and follow infection control practices.
  • Perform other duties as assigned.
