The Speech-Language Pathologist (SLP) will be responsible for identifying and assessing patients across various settings, including inpatients, outpatients, home care/hospice, and sub-acute care, serving patients from neonates to geriatrics. The SLP will perform age-specific assessments to gather data relevant to individual patient needs, focusing on speech, language, cognitive/communication disorders, and swallowing dysfunction as prescribed by a physician. Key responsibilities include developing and implementing treatment plans, evaluating patient responses to treatment, and assisting with discharge planning. The SLP will utilize equipment and adapt treatment strategies to improve swallow function and communication potential. They will also provide education on diagnosis, prognosis, and care plans to patients, families, and healthcare teams, helping patients achieve their maximum functional level. Additionally, the SLP will coordinate, delegate, and supervise supportive staff, potentially participate in community engagement activities, and contribute to the operational and performance improvement aspects of the department. Maintaining awareness of budgetary responsibilities, ensuring accurate and timely documentation and charge entry, and participating in all mandated educational programs are also crucial aspects of this role.
