About The Position

Elite Home Health is a Medicare-certified home health agency seeking dedicated Speech-Language Pathologists (SLPs) to provide high-quality, compassionate care to patients in their homes across Florida. The role focuses on helping patients improve communication, cognition, swallowing function, and overall independence. Requirements

Master’s degree in Speech-Language Pathology from an accredited program required.
Current state Speech-Language Pathologist (SLP) licensure in good standing required.
Minimum of one (1) year of clinical experience in speech therapy required.
Current CPR certification required.
Valid driver’s license, reliable transportation, and auto insurance.
Ability to pass background screening and all compliance requirements.

Nice To Haves

Home health, acute care, or rehabilitation experience preferred.
Certificate of Clinical Competence (CCC-SLP) preferred, if applicable.

Responsibilities

Perform initial and ongoing patient evaluations to assess speech, language, cognitive, voice, and swallowing disorders.
Complete comprehensive assessments and participate in OASIS-related coordination as applicable.
Develop, implement, and update individualized speech therapy treatment plans.
Ensure physician orders are appropriate and aligned with the patient’s therapy needs.
Provide skilled speech therapy interventions in accordance with physician orders and established care plans.
Treat speech, language, communication, cognitive, and swallowing impairments.
Support safe swallowing function and dysphagia management in accordance with clinical standards.
Promote restoration of communication abilities, cognitive function, and safe oral intake in the home setting.
Teach and counsel patients and caregivers regarding therapy goals, communication strategies, swallowing precautions, and cognitive support techniques.
Evaluate the home environment and identify strategies to improve safety and functional communication.
Coordinate additional services or referrals when appropriate to support patient outcomes.
Complete timely and accurate clinical documentation, progress notes, reassessments, and discharge summaries.
Evaluate effectiveness of therapy interventions and update the plan of care as needed.
Maintain compliance with Medicare requirements, state regulations, and agency policies.
Communicate regularly with physicians, nurses, therapists, and interdisciplinary team members regarding patient progress and needs.
Participate in care conferences, discharge planning, staff meetings, and required in-services.
Support continuity of care through coordinated interdisciplinary communication.
