About The Position

Elite Home Health is a Medicare-certified home health agency seeking dedicated Speech-Language Pathologists (SLPs) to provide high-quality, compassionate care to patients in their homes across Florida. The role focuses on helping patients improve communication, cognition, swallowing function, and overall independence. Requirements

  • Master’s degree in Speech-Language Pathology from an accredited program required.
  • Current state Speech-Language Pathologist (SLP) licensure in good standing required.
  • Minimum of one (1) year of clinical experience in speech therapy required.
  • Current CPR certification required.
  • Valid driver’s license, reliable transportation, and auto insurance.
  • Ability to pass background screening and all compliance requirements.

Nice To Haves

  • Home health, acute care, or rehabilitation experience preferred.
  • Certificate of Clinical Competence (CCC-SLP) preferred, if applicable.

Responsibilities

  • Perform initial and ongoing patient evaluations to assess speech, language, cognitive, voice, and swallowing disorders.
  • Complete comprehensive assessments and participate in OASIS-related coordination as applicable.
  • Develop, implement, and update individualized speech therapy treatment plans.
  • Ensure physician orders are appropriate and aligned with the patient’s therapy needs.
  • Provide skilled speech therapy interventions in accordance with physician orders and established care plans.
  • Treat speech, language, communication, cognitive, and swallowing impairments.
  • Support safe swallowing function and dysphagia management in accordance with clinical standards.
  • Promote restoration of communication abilities, cognitive function, and safe oral intake in the home setting.
  • Teach and counsel patients and caregivers regarding therapy goals, communication strategies, swallowing precautions, and cognitive support techniques.
  • Evaluate the home environment and identify strategies to improve safety and functional communication.
  • Coordinate additional services or referrals when appropriate to support patient outcomes.
  • Complete timely and accurate clinical documentation, progress notes, reassessments, and discharge summaries.
  • Evaluate effectiveness of therapy interventions and update the plan of care as needed.
  • Maintain compliance with Medicare requirements, state regulations, and agency policies.
  • Communicate regularly with physicians, nurses, therapists, and interdisciplinary team members regarding patient progress and needs.
  • Participate in care conferences, discharge planning, staff meetings, and required in-services.
  • Support continuity of care through coordinated interdisciplinary communication.
