About The Position

We are seeking a PRN Speech Language Therapist to join a team dedicated to enriching the lives of seniors through compassionate care and innovative programs. This role will provide exceptional services within an outpatient clinic, fostering a positive, supportive environment for residents, families, and team members on a PRN basis. Requirements

• A minimum of a Master’s Degree in Speech Language Pathology.
• CPR certified.
• Licensed by the State in the designated geographic area in their respective therapy profession.

Responsibilities

• Responsible for the maintenance and accuracy of records regarding patient care for all patients on personal case load.
• Responsible for the correct reporting of billing information of Speech Therapy regarding all patients on personal case load.
• Provide direct patient care as prescribed, including evaluation and treatment planning.
• Responsible for the reporting and interpretation of treatment results.
• Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and Therapy Management Corporation’s policies and procedures.
• Participate in meetings regarding patient care as appropriate.
• Maintains a high standard of resident care.
• Provide education to patient’s family as well as staff on the specific treatment plan as well as therapy goals for the patient.
• Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.
