About The Position

Hometown Home Health is seeking a compassionate and motivated Speech-Language Pathology Assistant (SLPA) to join our growing therapy team. This role supports patients in the home health setting by providing rehabilitative speech and language services under the direction of a supervising Speech-Language Pathologist and physician-directed plan of care. The ideal candidate is patient-focused, organized, and passionate about helping individuals improve communication, cognitive, and swallowing abilities while maintaining dignity and quality of life. Requirements

  • Active Florida Speech-Language Pathology Assistant (SLPA) license required
  • Must meet educational requirements outlined by Florida state law and Medicare Conditions of Participation
  • Current Basic Life Support (BLS) certification required
  • Ability to work independently while maintaining strong communication with supervising clinicians and team members
  • Strong verbal and written communication skills
  • Comfortable utilizing tablets, EMR systems, and related technology for documentation
  • Valid driver’s license, reliable transportation, and ability to travel locally for patient visits
  • Ability to meet the physical demands of patient care visits, including bending, lifting assistance, and prolonged standing or walking as needed

  • Home health experience preferred, but not required

Responsibilities

  • Provide speech and language therapy services under the supervision of a licensed Speech-Language Pathologist and according to the patient’s plan of care
  • Assist in evaluating patient needs and implementing appropriate therapy interventions
  • Educate patients, families, and caregivers on therapy exercises, communication strategies, and safe practices
  • Document patient progress, activities, and clinical findings accurately and timely within the medical record
  • Monitor patient conditions and communicate any changes to the supervising therapist and physician/provider as appropriate
  • Collaborate with interdisciplinary healthcare team members to ensure coordinated patient care
  • Participate in patient conferences, staff meetings, orientation, and continuing education activities
  • Maintain compliance with all applicable laws, regulations, agency policies, and professional standards
  • Travel to patient homes throughout the service area to provide care and support
