About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health SLP (Speech Language Pathologist) to work with our Home Health team to deliver individualized care to patients in their homes in south Pinellas county (including St. Petersburg, Clearwater, and surrounding areas). We provide skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support. This is a PRN role with flexible scheduling, allowing you to choose when and where you want to work, with 5-10+ home health visits per week available in South Pinellas county.

Requirements

  • Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
  • Active Florida SLP license

Nice To Haves

  • Home health care experience preferred
  • Experience using Homecare Homebase (HCHB) EMR is a plus

Responsibilities

  • Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to identify the client’s level of functioning, swallowing, and communication ability.
  • Develop a plan of care in consultation with the physician and other care team members.
  • Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
  • Instruct patients and families on using special educational strategies and techniques to improve sensory-motor development, perception, memory, language, and cognition.
  • Prepares clinical and progress notes of services provided, patient activities and condition and submit in a timely manner.
  • Collaborate with the interdisciplinary team to ensure seamless, holistic care
