Assumes full responsibility for providing quality speech language pathology services in compliance with ASHA and SLP scope of practice. Independently provides comprehensive services to improve patient participation, independence, and functioning through the evaluation of medical necessity for skilled speech therapy services, evidence-based therapeutic interventions, multidisciplinary team Involvement, counseling, referrals, and education. The Speech Language Pathologist will independently evaluate and diagnose functional abilities/disabilities under provider order within the areas of speech, language, voice, cognition, feeding and swallowing, fluency, resonance, and auditory habilitation/rehabilitation through standardized and informal testing procedures. Establishes patient-centered treatment goals in collaboration with the patient, family, and care team. Establishes written treatment plan and administers treatment for each patient according to stated goals. Documents evaluation and treatment of all patients in compliance with department standards, ASHA code of ethics, and medical-legal regulations. Provides education and instruction to the patient and family regarding evaluation results, plan of care, recommended interventions, home exercise programs. Including providing written instructions, as necessary. Provides culturally responsive care by creating a culturally safe environment, utilizing appropriate assessment and treatment materials, and obtaining language interpretation as needed.
