About The Position

FOX Rehabilitation is expanding in Ames, IA, and surrounding areas, seeking licensed Speech Language Pathologists to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. Utilizing the Geriatric House Calls™ model, you will help patients enhance mobility, independence, and overall quality of life. This role is situated within one of our Senior Living Community partners and is ideal for SLPs who value autonomy, flexibility, and achieving meaningful patient outcomes, with the freedom to manage their own schedules and build strong patient relationships. The role may also incorporate telehealth and virtual care options to increase flexibility, ensure continuity of care, and support a larger patient base. This is a high-impact SLP position focused on positively influencing the lives of the communities served.

Requirements

Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
Degree from an accredited Speech Language Pathology program
Basic computer and EMR proficiency
Current CPR certification

Responsibilities

Provide one-on-one Speech Language Pathology to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
Document effectively within FOX’s EMR system in accordance with regulatory standards
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
Provide telehealth / virtual care when appropriate to extend access and flexibility
