The Speech Language Pathologist is responsible and accountable for residents’ evaluation, ongoing assessment, planning and implementation of therapy interventions. They develop and revise the plan of care based on the residents’ needs and establish functional goals, outcomes, and discharge plans. The role also involves planning for residents’ discharge and anticipating their long-term and/or short-term needs. Additionally, the Speech Language Pathologist educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
