As a Home Health Speech-Language Pathologist (SLP) with Interim HealthCare, you’ll provide personalized, one-on-one therapy services to patients in the comfort of their own homes, where care is individualized and outcomes are meaningful. We are currently seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our Manchester, NH team. Requirements

  • Master's or Doctoral degree in Speech-Language Pathology
  • Current Speech-Language Pathologist (SLP) license in the State of New Hampshire
  • Current CPR certification required
  • Strong communication, clinical assessment, and interpersonal skills
  • Compassionate, dependable, and patient-focused approach to care

Nice To Haves

  • Minimum of one (1) year of speech therapy experience preferred
  • Home health experience preferred, but not required
  • Knowledge of state and federal home health regulations preferred

Responsibilities

  • Provide home-based speech therapy services to patients experiencing communication, cognitive, or swallowing disorders due to illness, injury, or medical conditions
  • Conduct patient evaluations and develop individualized treatment plans based on patient needs and goals
  • Assess speech, language, cognitive, and swallowing abilities
  • Implement therapeutic exercises and interventions focused on improving communication, cognition, fluency, articulation, and swallowing function
  • Monitor patient progress and modify treatment plans as needed
  • Maintain accurate and timely clinical documentation
  • Educate patients and family members regarding therapy goals, treatment plans, safety practices, and home exercise programs
  • Collaborate with physicians, nurses, therapists, caregivers, and other healthcare professionals to support patient outcomes
