About The Position
Concept Rehab is a therapist-owned and operated company with over 45 years of experience providing high-quality care in Post-Acute / Skilled Nursing Facility (SNF) settings. They offer a supportive, growth-focused environment for clinicians. This role involves evaluating patients, developing individualized treatment plans, providing therapy for communication, cognition, and swallowing disorders, collaborating with healthcare teams and families, maintaining documentation, and educating staff and caregivers.
Requirements
- Master’s degree in Speech-Language Pathology
- Active state licensure and ASHA certification (new graduate/CFY candidates welcome to apply)
- Strong communication, clinical judgment, and patient-focused mindset
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapy for communication, cognition, and swallowing disorders
- Collaborate with physicians, care teams, and families
- Maintain accurate documentation, billing, and discharge planning
- Educate staff and caregivers on restorative programs
