About The Position

This is a part-time, per diem role primarily for weekends at Advocate South Suburban and Advocate Trinity hospitals in the South Chicago area. The position requires fulfilling 1-2 shifts per month, with occasional weekday availability as desired but not required. Shifts are during the day.

Requirements

  • Master’s degree in Speech Pathology from an accredited school or educational requirements commensurate with the date of graduation.
  • Speech Language Pathologist license issued by the state in which the teammate practices (or licensure in at least one supported state prior to hire if supporting multiple states).
  • Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) American Heart Association.
  • Previous SLP experience preferred.
  • CCC SLP Required.
  • Intermediate computer skills.
  • Excellent communication, interpersonal, and organization skills.
  • Excellent organization, decision-making, and problem-solving skills.
  • Successfully completes all competency assessments/requirements pertaining to provision of patient care and consistent with site standards.
  • Maintains annual educational requirements for hospital-based certifications.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
  • Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs.

Responsibilities

  • Interprets physician referrals and conducts speech and language evaluations for patients with various disorders.
  • Establishes and implements individualized treatment plans based on evaluation results and functional goals.
  • Monitors patient progress, adjusts treatment plans, and communicates updates to physicians and teammates.
  • Identifies and addresses patient comfort factors.
  • Plans and completes patient discharge, including home programs, patient/family teaching, and referrals.
  • Communicates and collaborates with stakeholders for optimal patient care.
  • Completes all required documentation, including evaluations, progress notes, and discharge summaries, adhering to department policies and accreditation standards.
  • Records patient care and progress, completes daily statistics, billing, and scheduling.
  • Applies insurance guidelines regarding authorization and visit numbers.
  • Maintains productivity standards and adheres to billing and documentation policies.
  • Maintains a clean and orderly work area, including cleaning up after patient treatment and putting away equipment.
  • Performs manual techniques, patient positioning, and transporting duties requiring lifting and pushing/pulling.
  • Demonstrates skill in using available modalities, exercise equipment, and devices.
  • Promotes patient safety by participating in safety initiatives and adhering to safety and infection control measures.
  • Participates in performance improvement activities to enhance patient care outcomes.
  • Manages assigned caseload, informs leadership of coverage needs, and assists with coverage as needed.
  • Pursues professional growth through continuing education, in-services, and professional literature.
  • Orients new staff to clinical and department policies.
  • Participates in the evaluation process of coworkers.
  • May supervise non-licensed personnel, delegating tasks and providing supervision while maintaining responsibility for the plan of care.
  • Supports the student clinical education program by monitoring and providing work direction to students.
