About The Position
Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) to join their team. This role offers flexibility, the opportunity for meaningful patient relationships, and the ability to control income. The position is located in Fort Walton Beach/Okaloosa County, FL, with territories designed to minimize drive time through clustered scheduling. The role operates on a pay-per-visit model with additional compensation for Medicare differentials and mileage reimbursement, providing consistent patient volume. Requirements
- Active, unencumbered Florida Speech-Language Pathologist license
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Home health experience preferred but not required
- OASIS/Medicare documentation experience preferred
Responsibilities
- Conduct speech, language, cognitive, and swallowing evaluations
- Develop individualized treatment plans based on physician orders
- Provide therapy for communication, cognition, and dysphagia
- Educate patients and caregivers to improve functional outcomes
- Collaborate with physicians and interdisciplinary teams
- Complete OASIS documentation as required
- Document visits using streamlined, AI-assisted technology
