About The Position

Health Center - Franklin, IN 46131 Indiana Masonic Home Compass Park is looking to add an Occupational Therapist for Home Health. Compass Park is a vast 320-acre community in the heart of Johnson County adjacent to Indianapolis. It is a vibrant community through all seasons and a place of beauty, rich in tall trees and winding footpaths. This job is for Columbus/Franklin/South Indy area. We are seeking a compassionate, skilled, and independent Part-Time Speech-Language Pathologist (SLP) to join our home health care team. In this role, you will provide comprehensive, one-on-one therapy to geriatric patients within the comfort and safety of their own homes. You will evaluate, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders typically associated with aging, stroke, dementia, and progressive neurological conditions. Your goal is to maximize each patient's functional independence, improve their quality of life, and empower their caregivers with effective management strategies.

Requirements

  • Master’s degree in Speech-Language Pathology from an accredited program.
  • Active and unencumbered Speech-Language Pathologist license in Indiana.
  • Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
  • Valid driver’s license, reliable transportation, and active automobile liability insurance to travel between patient residences.
  • Ability to travel to patient homes, lift/transfer therapy materials, and adapt to various home environments
  • Strong clinical reasoning and independent problem-solving abilities in geriatric care.
  • Exceptional time management and organizational skills to handle a part-time mobile caseload.
  • Excellent verbal and written communication skills for patient education and team collaboration.
  • Proficiency with Electronic Medical Record (EMR) software and mobile charting platforms

Nice To Haves

  • Minimum of 1 year of clinical experience as an SLP preferred
  • Previous experience working with the geriatric population in home health, SNF, or acute rehab settings is highly desirable.

Responsibilities

  • Conduct comprehensive in-home assessments of speech, language, voice, cognition, and swallowing capabilities in older adults.
  • Develop, implement, and update individualized, goal-oriented plans of care based on physician orders and diagnostic findings.
  • Assess swallowing function, implement safety protocols, and recommend appropriate dietary textures to mitigate aspiration risks.
  • Deliver evidence-based therapy to address deficits caused by stroke, traumatic brain injury, dementia, Parkinson’s disease, or other age-related medical conditions.
  • Educate patients, family members, and caregivers on compensatory strategies, communication techniques, and home exercise programs.
  • Complete timely and accurate clinical notes, progress reports, and discharge summaries in compliance with Medicare, state, and agency guidelines.
  • Coordinate care with physicians, nurses, physical therapists, occupational therapists, and medical social workers to optimize patient outcomes.
