About The Position

CBI Home Health supports people at home and in the community, operating as one of Canada’s leading specialized community services, home and integrated care providers across the country. Our teams deliver care to help people live safely, independently, and with dignity. CBI Home Health is looking to hire a Speech Language Pathologist in Hawkesbury. This role is within the Integrated Care Program, focusing on post-acute recovery by providing personalized, impactful care in the community. The Speech Language Pathologist will support communication and swallowing where patients feel most comfortable—at home—working alongside a collaborative team.

Requirements

  • Registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario
  • Valid driver's license and vehicle insurance
  • Focus on embracing person and family-centered care
  • Comfortable delivering care both in person and using virtual methods/technology
  • Passionate, empathetic, use good judgement and apply critical thinking when making client decisions
  • Excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
  • Flexible and have demonstrated skills in critical thinking that you can apply to a variety of situations
  • Value the role a therapist plays in keeping clients at home, where they want to be

Responsibilities

  • Assessment and implementation of SLP interventions (i.e. swallowing assessments, dysphagia management, communication, etc.) in a home-based setting
  • Use of a collaborative and interdisciplinary team-based approach to care and services
  • Support a holistic and restorative approach to care (i.e. enhanced stroke pathway; cognitive communication supports, etc.)
  • Leverage our digital health platforms to support care delivery, collaboration, documentation, and intervention
