Speech Language Pathologist (SLP) - [Creston, IA] Blue Stone Therapy is seeking a Speech Language Pathologist (SLP) to provide therapy services to adult and geriatric patients at [Accura HealthCare of Creston] in [Creston, IA]. This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Speech Language Pathologist is responsible for assessments, care plan development, and implementation of treatment programs addressing dysphagia, swallowing disorders, cognitive-communication deficits, voice disorders, and speech and language impairments. This position will serve multiple partner communities, including [Corning Specialty Care] and [Bedford Specialty Care], with travel between locations as needed.

Requirements

  • Master’s degree in Speech Language Pathology from an accredited program
  • Current Speech Language Pathologist license (or eligibility for licensure) in applicable state
  • Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
  • Strong clinical reasoning, communication, and organizational skills
  • Commitment to patient-centered care and professional standards

Responsibilities

  • Perform speech therapy evaluations and develop individualized plans of care
  • Provide skilled speech language pathology services focused on functional and measurable outcomes
  • Treat swallowing disorders, dysphagia, cognitive deficits, communication disorders, and voice impairments
  • Participate in discharge planning and development of maintenance programs
  • Supervise Speech Language Pathology Assistants where applicable and in accordance with licensure standards
  • Maintain appropriate patient schedules per established guidelines
  • Complete timely, accurate documentation to support quality care and reimbursement requirements
  • Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
