About The Position
Functional Pathways is currently hiring at our Westminster Place location in Evanston, IL for a PRN Speech-Language Pathologist. The SLP shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.
Requirements
- Master's degree in Speech-Language Pathology from an accredited program
- ASHA (CCC) Certification
- Active state license
- Prior experience working as a Speech Therapist
- Experience in geriatric care
- Experience addressing functional limitations
- Experience addressing mobility challenges
- Experience addressing rehabilitation needs
Responsibilities
- Screen patients
- Evaluate patients
- Re-evaluate patients
- Develop discharge plans
- Revise established plans of care under physician supervision
