About The Position

Amazing Care Home Health Services is seeking a Speech Language Pathologist to join their team in the Logan area. The role involves implementing treatment plans for clients of all ages with cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning challenges. This includes performing evaluations, interpreting assessment results, developing plans of care with skilled interventions and goals, and creating and implementing speech and language treatment plans in conjunction with the physician. The SLP will assist pediatric and adult patients in developing or regaining functioning and improving their independence and quality of life. The position also requires coordinating care with referring physicians and other healthcare personnel, instructing clients, families/caregivers, and other healthcare personnel on treatment plans, identifying client and family/caregiver needs for other services and referring as appropriate, preparing and submitting documentation per organization policy, and maintaining clinical competency. Mileage is provided for commuting between patient visits.

Requirements

  • A master's or doctoral degree in speech-language pathology (SLP)
  • ASHA Certification or documentation of passing PRAXIS exam and completion of a Clinical Fellow/Externship
  • Speech-Language Pathologist (SLP) License in the state of Utah
  • Active BLS certification
  • Valid driver's license and auto insurance

Nice To Haves

  • A minimum of 1-year clinical experience in Speech Language Pathology is preferred, but not required

Responsibilities

  • Implement treatment plans to assist clients of all ages with cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning.

  • Coordinate care with referring physician and other health care organizations and personnel.
  • Instruct client, family/caregiver, and other organization health care personnel in the client's treatment plan.
  • Identify client and family/caregiver needs for other services and refers as appropriate.
  • Prepare and submit documentation per organization policy.
  • Maintain clinical competency in the theory and practice of speech-language pathology.
