About The Position

This is a full-time, field-based Speech Language Pathologist (SLP) role within the Twin Cities Metro Area, offering a high level of autonomy and the ability to manage your own schedule. The position focuses on delivering one-on-one in-home speech, language, and swallowing therapy, emphasizing meaningful patient outcomes over visit volume. The role is with Home Health Care, Inc., an established home care organization with over 30 years of experience, known for its clinician-first support and flexibility.

Requirements

• Active Minnesota SLP license.
• Reliable transportation for field-based visits.
• Comfort working independently in a home setting.
• A mindset focused on outcomes, relationships, and patient impact.

Responsibilities

• Deliver in-home speech, language, and swallowing therapy.
• Evaluate patients and build individualized, functional care plans.
• Focus on meaningful outcomes, not rushed visit volumes.
• Educate patients and families to support long-term success.
• Complete documentation in EMR.
