About The Position

Mozaic Senior Life is seeking a compassionate and experienced Speech-Language Pathologist (SLP) to provide skilled speech therapy services to clients in their homes. The SLP will evaluate, treat, and document speech, language, cognitive-communication, and swallowing needs while working collaboratively with clients, families, caregivers, physicians, and the interdisciplinary care team. This is a per diem position with flexibility to accommodate client and departmental needs.

Requirements

  • Master's degree in Speech-Language Pathology from an accredited college or university.
  • Minimum of 1 year of clinical experience in long-term care, acute care, rehabilitation, home care, or a related setting.
  • Current Connecticut Speech-Language Pathology license.
  • Current ASHA Certificate of Clinical Competence (CCC-SLP) or eligibility, as applicable.
  • Valid driver's license and reliable transportation.
  • Strong clinical, communication, organizational, and documentation skills.
  • Ability to work independently while functioning effectively as part of an interdisciplinary team.

Nice To Haves

  • Experience working with older adults and individuals with speech, language, cognitive, and/or swallowing disorders preferred.

Responsibilities

  • Provide professional speech-language pathology evaluations and treatments in clients' homes.
  • Evaluate and treat speech, language, cognitive-communication, voice, and swallowing disorders related to injury, surgery, acute or chronic illness, or other medical conditions.
  • Develop and implement individualized treatment plans based on client needs and goals.
  • Educate clients, families, caregivers, and healthcare professionals on strategies and techniques to maximize function, communication, independence, and safety.
  • Monitor client progress and adjust treatment plans as appropriate.
  • Communicate effectively with physicians and members of the interdisciplinary team regarding client status and treatment recommendations.
  • Complete all required clinical documentation accurately and within established timeframes.
  • Accurately record billable treatment time in accordance with Medical Home Care procedures.
  • Demonstrate flexibility in scheduling to meet client and departmental needs.
  • Maintain current knowledge of evidence-based practices and professional standards in Speech-Language Pathology.
  • Provide compassionate, respectful, and person-centered care to older adults and individuals with complex medical needs.
