About The Position

VNA & Hospice of the Southwest Region is seeking compassionate and motivated Speech Language Pathologists (SLPs) to join our team in Rutland County. We are hiring for part-time and/or per diem positions working with adult and/or pediatric patients in the home health setting. Based in Vermont, we are one of the largest non-profit home health and hospice care agencies delivering a wide range of advanced medical care with compassion, dependability, and expertise to people of all ages. Requirements

  • Master's Degree in Speech-Language Pathology from an accredited program.
  • Current licensure as a Speech-Language Pathologist in the state of practice.
  • Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA) required for adult.
  • Valid driver's license, auto insurance, and reliable transportation.

  • Home health experience a plus.

Responsibilities

  • Performs comprehensive (OASIS and SLP) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.
  • Actively manages and adjusts patient schedules to meet productivity goals and patient care needs, including coordinating schedule changes and communicating updates to supervisors.
  • Performs initial and ongoing assessments of patients' communication, cognitive, developmental, and/or swallowing status.
  • Develops and implements individualized treatment plans based on physician orders and patient needs.
  • Provides direct speech, language, voice, fluency, cognitive, and swallowing therapy services in the patient's home.
  • For pediatric patients, supports speech/language development, feeding and swallowing skills, and caregiver education.
  • For adult patients, addresses communication, cognition, and swallowing deficits related to stroke, neurological conditions, injury, or illness.
  • Instructs and educates patients, caregivers, and family members regarding goals and home exercise programs.
  • Collaborates with interdisciplinary team members (nursing, PT, OT, social work) to ensure holistic and coordinated care.
  • Monitors and documents progress toward established goals and adjust plans as necessary.
