About The Position

At Qualicare Home Health, we empower Speech-Language Pathologists to deliver one-on-one care in patients’ homes—helping patients improve communication, cognitive function, and swallowing safety in a supportive and collaborative environment. This role offers a flexible schedule, a supportive interdisciplinary team, and streamlined documentation with EMR (WellSky). We provide training and support for clinicians transitioning into Home Health and offer mileage reimbursement, referral bonuses, and recognition programs.

Requirements

• Active California Speech-Language Pathology license
• Master’s Degree in Speech-Language Pathology from an accredited program
• Strong communication, documentation, and patient education skills
• Valid California driver’s license and reliable transportation

Nice To Haves

• CCC-SLP preferred
• Home Health or adult rehabilitation experience preferred, not required

Responsibilities

• Perform comprehensive speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on patient needs and clinical goals
• Provide therapy interventions to improve communication and swallowing safety
• Educate patients and caregivers on communication strategies, swallowing precautions, and home exercises
• Collaborate closely with interdisciplinary care team members
• Monitor patient progress and modify treatment plans as needed
• Maintain timely and accurate documentation in EMR (WellSky)
