About The Position

Concept Rehab is a therapist-owned and operated company with over 45 years of experience providing high-quality care in Post-Acute / Skilled Nursing Facility (SNF) settings. We offer a supportive, growth-focused environment for clinicians. As a Speech Language Pathologist (SLP), you will evaluate patients, develop individualized treatment plans, and provide therapy for communication, cognition, and swallowing disorders. You will collaborate with physicians, care teams, and families, maintain accurate documentation, and educate staff and caregivers on restorative programs.

Requirements

• Master’s degree in Speech-Language Pathology
• Active state licensure and ASHA certification (new graduate/CFY candidates welcome to apply)
• Strong communication, clinical judgment, and patient-focused mindset

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapy for communication, cognition, and swallowing disorders
• Collaborate with physicians, care teams, and families
• Maintain accurate documentation, billing, and discharge planning
• Educate staff and caregivers on restorative programs
