About The Position
FOX Rehabilitation is expanding in Folsom, CA, and is seeking dedicated, licensed Speech Language Pathologists to provide exceptional one-on-one care to older adults in their homes and within Senior Living Communities. Utilizing the Geriatric House Calls™ model, you will help patients enhance their mobility, independence, and overall quality of life. This role involves working in two partner Senior Living Communities: Prairie City Landing and Sunrise of Fair Oaks. The position is ideal for Speech Language Pathologists who value autonomy, flexibility, and achieving meaningful patient outcomes, with the ability to manage their own treatment schedules and foster strong patient relationships. Clinicians may also integrate telehealth and virtual care options, offering increased flexibility, better care continuity, and the capacity to assist more patients. This is a high-impact SLP role for those looking to make a positive difference in the lives of the communities served.
Requirements
- Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Speech Language Pathology program
- Basic computer and EMR proficiency
- Current CPR certification
- Current Certificate of Clinical Competence (ASHA CCC)
Responsibilities
- Provide one-on-one Speech Language Pathology to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
- Document effectively within FOX’s EMR system in accordance with regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
