About The Position

Performs appropriate patient assessments resulting in an effective treatment plan. Reassesses patients at appropriate intervals of time demonstrating the ability to observe treatment effect and modify the plan of care as indicated. Involves the patient and /or family in the assessment process and the formation of the care plan. Provides care in accordance with physician’s orders. Demonstrates appropriate case management to ensure quality of care and best patient outcome. Communicates appropriately to physicians, nurses, case managers, and other members of the health care team to ensure care is collaborative and appropriate. Assesses and documents patient outcomes criteria at the initial and discharge evaluations. (all shoulder, back, incontinence, and anodyne). Demonstrates knowledge of speech therapy modalities / treatments and performs correctly. Demonstrates knowledge of and competency in using equipment pertinent to providing speech therapy services. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning. Includes the family in teaching as appropriate. Documentation meets current standards and policies. Documentation is legible. No inappropriate abbreviations are used. Documentation is completed in a timely manner. Medical Records are closed as per policy. Time is adequately documented in the assessments and daily treatment notes. Each charge bill adequately reflects all services provided.

Requirements

  • North Carolina and American Speech / Language Association Certification
  • Must be a High School or GED Diploma Graduate.
  • Masters in Communication Disorders
  • Must be able to use visual near and distance acuity.
  • Must have verbal ability to communicate in person and over the phone or radio.
  • Must be able to assist co-workers in transfers and transport supplies, patients/residents, or equipment using good body mechanics.
  • Must have functional range of motion of the cervical, thoracic and lumbar spines, upper and lower extremities with a grip strength of 60-80 pounds, specific to the job class evaluation.
  • Must be able to forward reach, overhead reach, bend, squat, kneel and change from one position to the other rapidly.
  • Must be able to lift 25-60 lbs. and carry 25-50 pounds specific to job class evaluation.
  • Must be able to tolerate high noise, busy atmospheres and noxious odors.
