The Speech–Language Pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement. This role involves conducting evaluations, assessing home environments, identifying equipment needs, and developing individualized plans of care in partnership with patients and caregivers. The Speech-Language Pathologist provides ordered services, patient and family counseling, and education. They prepare clinical notes, communicate with physicians and other healthcare practitioners, and identify functional speech defects to create improvement plans. The role also includes administering therapy programs, recommending communication devices, participating in case conferences, and maintaining clinical competency. Additionally, the Speech-Language Pathologist identifies needs for other services and refers appropriately, may supervise home health aides in specific procedures, and participates in quality improvement programs.
