About The Position

Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) for PRN positions. The position involves 1:1 patient care in the home within specified counties in Florida. The company utilizes technology to streamline charting and provides growth and clinical support, including free CEUs.

Requirements

• Active, unencumbered Florida Speech-Language Pathologist license
• Valid driver’s license, reliable transportation, and auto insurance
• Employment is contingent upon successful completion of an AHCA Level 2 background screening.

Nice To Haves

• Home health experience preferred but not required
• OASIS/Medicare documentation experience preferred

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy for communication, cognition, and dysphagia
• Educate patients and caregivers to improve functional outcomes
• Collaborate with physicians and interdisciplinary teams
• Complete OASIS documentation as required
• Document visits using streamlined, AI-assisted technology
