About The Position

Flagstar Nursing is seeking Speech Language Pathologists (SLP) for Skilled Nursing Facilities. We are currently staffing for the Rehab Department in state-of-the-art SNFs throughout NYC and Long Island. Flagstar Rehab SLPs collaborate with a diverse, multi-disciplinary team to conduct evaluations, provide treatments, and educate patients and their families to enhance speech and swallowing capabilities and improve daily functioning. This position is not accepting Clinical Fellows (CFY) at this time. Flagstar Rehab offers the choice of Direct Employment with our agency or the option to work as an Independent Contractor. We can also assist with setting up a corporation free of charge.

Requirements

• Not accepting Clinical Fellows (CFY) at this time.
• Current NYS SLP License and Registration
• Clinical Competence Certification (CCC)
• CPR Certificate
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
