About The Position

Hanford Post Acute is seeking a passionate Speech Language Pathologist (SLP) to join their dedicated team. The role involves providing high-quality, person-centered care in a skilled nursing setting, focusing on helping patients regain communication and swallowing abilities. The SLP will evaluate, plan, and implement therapy services to meet the communication and swallowing needs of residents.

Requirements

  • Bachelor’s degree in Speech-Language Pathology required
  • Active and unencumbered California SLP license
  • CPR certification
  • Continuing education per state requirements
  • Strong clinical reasoning and interpersonal communication skills
  • Proficient in electronic documentation systems (e.g., Casamba, PCC)

Nice To Haves

  • Master’s or Doctorate preferred
  • Experience in SNF setting preferred

Responsibilities

  • Evaluate patients within 24 hours or 1 business day of physician referral (with documentation if delayed)
  • Develop and implement personalized treatment plans approved by referring physician
  • Provide direct treatment and document patient progress in compliance with state and facility standards
  • Supervise SLP Assistants and Clinical Fellows as needed
  • Participate in interdisciplinary meetings, care conferences, and in-service training
  • Provide caregiver education and discharge planning
  • Maintain accurate and timely documentation in systems like Casamba and PointClickCare
  • Comply with all applicable laws, regulations, and facility policies
