About The Position
FOX Rehabilitation is hiring licensed Speech Language Pathologists in Yonkers, New York, to provide one-on-one care to older adults in private residences and senior living communities. Using the Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other virtual services. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and work in a clinician-operated, interdisciplinary practice. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. Full-time roles are salaried with benefits; part-time benefits eligibility is based on minimum hours, and PRN compensation is per unit. Compensation depends on experience and employment type. The stated New York base pay range for full-time roles is $70,000–$105,000 annually, with an opportunity to earn more; part-time and PRN compensation is prorated. Requirements
- Hold a valid Speech Language Pathology license in the state(s) of practice, or be eligible to obtain one.
- Have a degree from an accredited Speech Language Pathology program.
- Have basic computer and EMR proficiency.
- Hold current CPR certification.
- Hold a current Certificate of Clinical Competence (ASHA CCC).
- Be able to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, repositioning patients, extended standing and walking, stair climbing, and therapeutic activities.
- Wear PPE when required.
Responsibilities
- Provide one-on-one Speech Language Pathology services to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
